NKHS is seeking a dedicated and detail-oriented Care Coordinator to support families with children and youth (birth to age 22) who have developmental disabilities through our Bridge Program. In this role, you will provide high-quality service coordination designed to promote stability, connect families with critical resources, and support meaningful outcomes for the individuals and families we serve. This is a temporary position with the possibility of becoming permanent based on program needs and performance. This position plays an important role in helping families navigate complex systems of care while ensuring children and youth receive the services and supports they need to thrive. About NKHS Northeast Kingdom Human Services (NKHS) is a 501(c)(3) private not-for-profit organization dedicated to providing high-quality, comprehensive community mental health services in Vermont’s beautiful Northeast Kingdom. As a Designated Agency contracted with the State of Vermont, NKHS provides services to individuals with intellectual and developmental disabilities, children and youth experiencing emotional and behavioral challenges, and adults facing mental health and substance use concerns. With more than 500 employees serving nearly 4,000 community members annually across Caledonia, Essex, and Orleans counties, NKHS offers a wide range of programs including case management, community and home supports, residential care, psychiatry, therapy, vocational supports, school-based counseling, emergency care, and respite services.
