Your Role as a Bridge Care Coordinator As a Bridge Care Coordinator , you will be an essential part of our team, providing support and companionship to aging patients through our Bridge Program .

Requirements

• 18+ years old
• Healthcare experience required
• Strong organizational & computer skills
• Ability to work independently
• Valid driver's license & clean driving record
• Reliable transportation & current auto insurance (MVR check required)
• Negative TB test
• Ability to pass a Background and Drug Screen.

Responsibilities

• Visiting patients in their homes and providing peace of mind.
• Educating families about available healthcare services.
• Connecting patients with community resources.
• Helping transition patients between care services, including hospice.
