About The Position

The Breast Oncology Care Coordinator is a multi-site role that works in conjunction with the Oncology Nurse Navigator(s) and the breast service line team to ensure access for individuals with suspected or confirmed breast cancer to quality care that is timely, coordinated and patient-centered.

Requirements

• Bachelor's Degree Healthcare or related field Required
• 2 years experience in a healthcare setting or equivalent preferably in oncology Required
• General Experience and knowledge of basic clinical concepts Required
• General Experience and knowledge of EPIC systems Required
• General Experience and knowledge of complex patient issues and social determinants of health Required
• Other within 6 months of hire completion of competency-based training and education in patient navigation from a qualifying training program Required
• Natl Accred Prg Breast Ctr Required

Nice To Haves

• General Experience and knowledge of patient outreach methods and strategies Preferred
• General Experience with medical terminology Preferred
• General Experience working as a patient navigator/community health worker Preferred
