Requirements

  • Graduate from an accredited School of Nursing
  • Licensed in the state as a Registered Nurse
  • 3–5 years Medicare-certified home health experience
  • Knowledge of Medicare and Medicaid guidelines
  • Criminal background check
  • Must have a current CPR (BLS) certification

Nice To Haves

  • CHAP survey experience preferred

Responsibilities

  • Oversee all skilled nursing services delivered under the Home Health Benefit
  • Ensure care is provided in accordance with physician orders, Plans of Care, and patient goals
  • Monitor, review OASIS assessments for accuracy, timeliness, and compliance
  • Ensure infection prevention, safety, and evidence-based clinical practice standards are met
  • Monitor clinical outcomes, rehospitalizations, adverse events, and patient satisfaction
  • Serve as the agency’s primary clinical adviser to staff, patients and business associates
  • Ensures full compliance with Medicare Conditions of Participation (CoPs), CHAP accreditation standards, and state regulations
  • Maintain constant survey readiness for CMS validation, state, and CHAP surveys
  • Participate in regulatory and accreditation surveys and corrective action plans
  • Ensure compliance with Face-to-Face encounter requirements
  • Ensure clinical documentation supports medical necessity and payor billing requirements
  • Support audits, ADRs, medical records requests and payer reviews
  • Participates in the QAPI program in alignment with corporate goals, CMS and CHAP standards
  • Analyze quality data, trends, and risk indicators
  • Implement performance improvement initiatives
  • Monitor Star Ratings, HHCAHPS, and HHVBP outcomes
  • Recruit, hire, supervise, train and retain qualified nursing staff
  • Ensure staff competency, licensure, and education compliance
  • Conduct performance evaluations and disciplinary actions
  • Promote a culture of accountability and patient-centered care
  • Collaborate with physicians, therapists, intake, QA, and leadership
  • Support referral acceptance decisions
  • Assist with staffing models and census management
  • Participate in leadership meetings and strategic planning
