About The Position

A Registered Nurse who assists the Administrator with the supervision of home health services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. Skilled nursing and other therapeutic services furnished are under the supervision of the Administrator/Alternate Administrator. The Supervising Nurse/Alternate Supervising Nurse/Branch Director is available at all times during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel.

Requirements

  • Registered nurse who is a direct employee of the agency, graduated from an approved school of nursing, licensed in the state of Texas or in accordance with the Texas Board of Nursing rules for Nurse Licensure Compact, and has at least one-year experience as a registered nurse within the last 36 months.
  • Knowledge and ability to apply community health principles and practices.
  • Knowledge of Agency policies and procedures.
  • Ability to supervise, guide and develop skills and performance of service personnel.
  • Ability to exercise independent judgment.
  • Ability to work with individuals.
  • Ability to enlist cooperation of many people in furthering a program.
  • Ability to assist in evaluating personnel according to the Agency policy.
  • Monitors probationary period for new employees.
  • Ability to deal effectively with high levels of stress.

Nice To Haves

  • Three to five years of experience in health care/home care, preferred.
  • Two years’ experience in a supervisory position, preferred.

Responsibilities

  • Understands and adheres to established Agency policies and procedures.
  • Participates in the development of standards which ensure safe and therapeutically effective service to patient/family.
  • Ability to supervise, guide and develop nursing skills and performances.
  • Consults with the Administrator/Alternate Administrator in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of health personnel.
  • Assists in recruiting, hiring, evaluating and terminating nursing personnel. Assists in instituting, interpreting and documenting disciplinary action.
  • Responsible for the orientation of new patient care personnel.
  • Assists in the planning and consultative needs of staff.
  • Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state and local laws.
  • Displays a willingness to support policies and procedures and uses appropriate channels for changes of such policies.
  • Assists in the coordination of interdisciplinary team services.
  • Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.
  • Participates in the coordination of Agency's services with the services of other community agencies.
  • Participates in studies and research and other administrative functions as assigned.
  • Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performances.
  • Observes confidentiality and safeguards all patient related information.
  • Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved.
  • Serves as a resource person to employees.
  • Develops a cooperative relationship and communicates effectively and professionally with physicians.
  • Investigates and reports any problem relating to patient care and/or employee well-being.
  • Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Clinical Services/Administrator.
  • Attends pertinent continuing education programs other than routine inservices and shares information with staff.
  • Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.
  • Assists with the Agency's ongoing Quality Assessment and Performance Improvement (QAPI) program.
  • Assists with the development, implementation and evaluation of patient plans of care.
  • Reviews patient medical records for compliance with federal, state, accrediting bodies, local and Agency policies and guidelines.
  • Assists Administrator/Alternate Administrator in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects.
  • Ensures that all necessary supplies and equipment are available.
  • Must be able to perform nursing functions in times of staff shortage.
  • Available to share on-call with nursing personnel as needed.
  • Ensures that documentation is completed in accordance with Agency policy.
  • Assists with staffing visit assignments and on-call.
  • Other duties as assigned by the Administrator/Alternate Administrator.
  • Be available to agency personnel at all times in person or by telephone
  • Participate in activities relevant to services furnished, including the development of qualifications and assignment of agency personnel.
  • Ensure that a client’s plan of care or care plan is executed as written.
  • Ensure that an appropriate health care professional performs a reassessment of a client’s needs when there is a significant health status change in the client’s condition, at the physician's request or after hospital discharge.
