About The Position

Serve as the Brain Health Clinical Coordinator responsible for supporting clinical and community implementation activities. Working as a program team member under the direction of the Tribal Dementia Program Director, coordinate technical assistance, education, quality improvement, data collection, and partnership activities with Tribal clinics, Tribal programs, caregivers, community members, and academic and clinical professionals. The position helps strengthen dementia-capable systems, early detection and referral practices, caregiver support, and culturally responsive brain health programming. Attendance as scheduled is an essential function of this position. This is an office-based position and is not remote. Frequent travel is required to meet the needs of the position. This position can be in our Green Bay, Wisconsin office, our Madison office, or our Grand Rapids, Michigan office. This position is not a remote position.

Requirements

  • Master’s degree in public health or another health-related field.
  • Clinical, hospital, clinic, long-term care, home health, public health, or community health experience is required.
  • Knowledge of clinical and community health systems and the ability to work effectively with Tribal clinic staff, Tribal programs, community members, caregivers, and other professionals.

Nice To Haves

  • Experience working with American Indian and Alaska Native people, Tribal governments, Tribal organizations, Tribal clinics, Tribal Epidemiology Centers, Urban Indian organizations, and/or not-for-profit agencies.
  • Experience in dementia care, gerontology, neurology, primary care, caregiving, healthy aging, chronic disease prevention, behavioral health, care coordination, federal grant implementation, technical assistance, training, or clinical workflow development.
  • Current or previous health-care credential or certification, such as CNA, LPN, RN, CHN, CHR, or another relevant credential.

Responsibilities

  • Coordinate clinical and programmatic activities in accordance with approved work plans, grant requirements, organizational policies, Tribal Dementia Program Director, and funder guidance.
  • Develop and maintain working relationships with Tribal clinic administrators, health-care providers, nurses, community health nurses, community health representatives, CNAs, aging and senior programs, caregivers, community members, Tribal leadership, academic partners, and public-health professionals.
  • Provide technical assistance to Tribal clinics and programs to strengthen dementia-capable systems, including early detection and screening workflows, referral processes, care coordination, caregiver education, resource navigation, and follow-up practices.
  • Assist Tribal clinics and programs in assessing dementia-related policies, procedures, workflows, workforce needs, services, and community resources; identify gaps and support practical quality-improvement plans.
  • Coordinate and deliver culturally responsive education and training on brain health, dementia risk reduction, signs and symptoms of dementia, early detection, caregiver support, communication, safety, and available resources.
  • Develop and maintain clinical and community resource materials, referral tools, educational handouts, presentations, implementation guides, and other program products.
  • Adapt evidence-informed information for Tribal audiences in collaboration with community partners while respecting Tribal sovereignty, local culture, community priorities, and the diversity of teachings and practices among Tribal Nations.
  • Plan and facilitate meetings, learning sessions, community events, workgroups, trainings, and presentations for clinical, program, community, and professional audiences.
  • Track grant activities, milestones, deliverables, participant reach, technical assistance, training outcomes, and partner engagement; maintain complete and accurate documentation.
  • Collect, review, summarize, and interpret quantitative and qualitative information for program monitoring, evaluation, quality improvement, health profiles, reports, presentations, and grant deliverables.
  • Apply appropriate statistical calculations to program findings and collaborate with epidemiology and evaluation staff to ensure data quality and appropriate interpretation.
  • Prepare written reports, summaries, business correspondence, meeting materials, health profiles, and contributions to semi-annual, annual, and other funder reports.
  • Promote responsible data stewardship and uphold Tribal data sovereignty, confidentiality standards, grant conditions, and applicable organizational requirements.
  • Respond to requests for information and technical assistance from Tribal and community partners and follow requests through to completion, including written and oral responses.
  • Represent GLITC and GLITEC professionally at Tribal, regional, state, and national meetings, conferences, training, and collaborative activities as assigned.
  • Follow directions from senior staff and Organization Leadership and integrate feedback and community perspectives into assigned work.
  • Participate in writing grant applications, continuation materials, progress reports, and other funding proposals.
  • Engage in networking activities to maintain awareness of clinical, caregiver, dementia, and healthy-aging resources available to Tribal communities.
  • Assure that services are delivered in a culturally responsive and respectful manner.
  • Participate on committees, advisory groups, and workgroups within and outside the organization as deemed necessary and appropriate by the supervisor.
  • Attend all required training sessions, staff meetings, conferences, and seminars and remain current on advances in dementia care, gerontology, brain health, public health, and relevant clinical guidance.
  • Assist with scheduling, travel coordination, procurement, contractor communication, and other operational tasks necessary to complete grant activities.
  • Perform other duties as necessary to fulfill the responsibilities of this position.
