About The Position

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.

Requirements

• A bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or other related human services field
• Must have a valid driver's license and own a reliable means of transportation

Nice To Haves

• Experience in the human services field working with children is preferred
• Bi-lingual a plus

Responsibilities
