About The Position

Bittersweet Encounters (BSE) is a women-led, minority-owned behavioral health and care coordination organization serving underserved communities throughout the Inland Empire and across California through telehealth services. Founded by a Licensed Marriage and Family Therapist, BSE provides compassionate, culturally responsive mental health, behavioral health, and care coordination services that support whole-person wellness for individuals and families. Psychotherapy is the foundation of our work, with expanded services including Enhanced Care Management (ECM), Community Health Worker support, and supervised visitation services for families involved in child welfare and family court systems. Bittersweet Encounters is committed to advancing health equity and improving outcomes for communities disproportionately impacted by health disparities. Bittersweet Encounters is seeking compassionate, organized, and community-oriented professionals to serve as Birth Equity Lead Care Managers (ECM & Outreach) supporting pregnant and postpartum Kaiser Permanente Medi-Cal members through a Birth Equity initiative. This role focuses on providing culturally responsive, trauma-informed care coordination, outreach, engagement, and resource navigation to support maternal health and family stability. The ideal candidate is passionate about serving underserved communities, skilled in relationship-building, and committed to whole-person care.

Nice To Haves

  • Bachelor’s degree in social work, psychology, public health, human services, or related field preferred.
  • Experience in ECM, CalAIM, care coordination, outreach, case management, maternal health, behavioral health, or community-based services preferred.
  • Experience working with Medi-Cal populations and underserved communities.
  • Knowledge of social drivers of health and community resource navigation.
  • Strong communication, engagement, and relationship-building skills.
  • Bilingual English/Spanish preferred.
  • Experience working with pregnant/postpartum populations, child welfare systems, or family support services is a plus.

Responsibilities

  • Provide Enhanced Care Management services to pregnant and postpartum Medi-Cal members.
  • Conduct outreach and engagement activities to support member enrollment, participation, and continuity of care.
  • Build trusting relationships with members through culturally responsive and trauma-informed engagement practices.
  • Complete comprehensive assessments and individualized care plans.
  • Coordinate medical, behavioral health, and community-based services.
  • Support members in addressing social drivers of health, including housing, food access, transportation, and healthcare navigation.
  • Collaborate with healthcare providers, community organizations, and interdisciplinary teams to support referrals and member care coordination.
  • Maintain ongoing communication and follow-up to support successful member engagement and retention.
  • Maintain timely and accurate documentation in accordance with ECM and organizational standards.
  • Participate in trainings, supervision, outreach activities, and team meetings.
