Requirements

  • Mississippi: Master’s degree in a human services, behavioral health, social work, counseling, or related field required.
  • Mississippi: Must meet payer-required credentialing standards, which may include: Licensed Social Worker (LSW), or DMH-approved Mental Health Therapist credential, or Registered Nurse (RN)
  • Mississippi: Must have 2 years of experience in mental health care.
  • Florida: Must hold a Bachelor’s degree in a human services discipline, including but not limited to: Social Work, Psychology, Sociology, Counseling, etc.
  • Florida: Must have experience working with children and families in case management or community resource capacity.

Responsibilities

  • Provides best practice, quality case management services.
  • Ensures that all documentation is completed per Payer, State, and COA standards and requirements.
  • Assists in marketing new referrals.
  • Communicates with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
  • Develops service plans with the input of the treatment team and the Clinical Supervisor.
  • Connects families with needed and available community resources.
  • Follows-up with clients and agencies as appropriate to document use/success of referral.
  • Completes documentation within established time frames.
  • Meets with clients weekly or bi-weekly for up to 12 months.
  • Attends and participates bi-weekly with Treatment Team meetings.
  • Serves as a liaison with other professionals and agencies related to the client.
  • Actively recruits new referrals to ensure that your caseload stays full.
  • Provides 24/7 on-call support for clients as per program requirements.
  • Ensures compliance with all state regulatory bodies.
