The Role We’re hiring a Bilingual/Spanish Speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. Clinical Navigators deliver proactive, patient-centered outreach and follow-through. Depending on program needs, this role includes structured patient check-ins, coaching and goal-setting, care coordination to close gaps in care, and addressing barriers such as access to resources, transportation, or medication refills. Clinical Navigators document patient interactions in the Cadence platform and collaborate closely with the clinical team to escalate concerns and coordinate next steps, ensuring patients feel supported, informed, and engaged throughout their care journey.
