About The Position

The Patient Enrollment & Outreach Specialist facilitates patient access to care by conducting eligibility and financial screenings, assisting with insurance and Sliding Fee enrollment, providing patient education, and connecting patients to available health center services in compliance with FQHC and HRSA requirements.

Requirements

  • Bilingual in English and Spanish
  • High School Diploma or equivalent
  • 1–2 years of healthcare enrollment, outreach, eligibility, or patient access experience
  • FQHC experience preferred
  • Experience with EHR/EPM systems, preferably NextGen
  • Proficient in Microsoft Word, Excel, and Outlook
  • Ability to perform prolonged computer work and lift up to 30 lbs as needed
  • Strong cultural competency and experience serving diverse populations
  • Knowledge of Medi-Cal, Medicare, and Covered California plans
  • Understanding of HRSA regulations, Sliding Fee programs, and enabling services
  • Experience with payer portals and eligibility verification systems
  • Strong organizational, communication, customer service, and problem-solving skills
  • Ability to multitask, meet productivity goals, and maintain compliance
  • Meet productivity and enrollment goals consistently
  • Reliable transportation for work-related travel

Responsibilities

  • Conduct patient outreach, registration, enrollment, and appointment scheduling
  • Assist with insurance enrollment, renewals, and eligibility verification
  • Complete financial screenings and HRSA complaint Sliding Fee applications
  • Educate patients on services, benefits, rights, and eligibility requirements
  • Reduce barriers to care through transport & translation assistance & referrals to support services
  • Maintain accurate patient records in EHR/EPM systems
  • Document outreach and enrollment activities in accordance with HRSA and UDS standards
  • Reconcile managed care rosters and coordinate corrections
  • Represent the organization at community outreach events
  • Collaborate with departments to support quality and engagement goals
  • Track and report outreach, enrollment, and productivity metrics
  • Ensure HIPAA compliance and adherence to organizational policies
  • Support process improvements and patient retention efforts
  • Promote financial sustainability through patient enrollment and retention
  • Monitor productivity, managed care reports, sliding fee audits, and renewal metrics
