About The Position

The Center for Human Development (CHD) is seeking a Bilingual Outreach Worker for its Adult Community Clinical Services (ACCS) program in Springfield, MA. ACCS is funded by the Department of Mental Health and employs a multidisciplinary outreach approach for individuals in the community who need psychiatric and other services. This role involves active engagement, including assertive outreach, consistent assessment and treatment planning, skill-building, symptom management, addiction treatment, and monitoring and support for behavioral and physical health. A generous sign-on bonus of $1,500 is offered. ACCS teams are multidisciplinary, including licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The team collaborates to deliver person-centered, evidence-based services aligned with individuals' treatment goals. The Direct Care Staff assists participants needing psychiatric services in developing the skills required for independent community living. This role operates within the Agency's team model, fostering open communication, support, and accountability among colleagues.

Requirements

  • At least one year experience working with population preferred
  • High School diploma required
  • Interested candidates should be Bilingual (English/Spanish). With ability to speak, read and write in both fluently

Nice To Haves

  • Bachelor’s degree from an accredited educational institution preferred (required for DC II, or additional experience)

Responsibilities

  • Assists participants with activities of daily living e.g. personal hygiene, meal preparation, housekeeping, laundry, money management, etc.
  • Transports participants in matters related to service delivery, e.g. medical appointments, shopping, job interviews, vocational program, banking, laundry
  • Administer medication accordance with MAP policies and procedures
  • Maintains records in accordance with standard format and timetables to include assessment and progress reports/service notes, incident reports, money disbursements, and other specialized documentation as required
  • Provides for and participates in recreational activities, e.g. dining out, spectator sports, movies, etc. for the participants
  • Participates in the development of treatment plans of those served
