About The Position

The Center for Human Development (CHD) is seeking a Bilingual Outreach Worker for its Adult Community Clinical Services (ACCS) program in Springfield, MA. ACCS is funded by the Department of Mental Health and utilizes a multidisciplinary outreach approach for individuals needing psychiatric and other services while living independently in the community. This role involves active engagement, including assertive outreach, consistent assessment and treatment planning, skill-building, symptom management, addiction treatment, and monitoring/support for behavioral and physical health.

Requirements

High School diploma required
Interested candidates should be Bilingual (English/Spanish). With ability to speak, read and write in both fluently

Nice To Haves

At least one year experience working with population preferred
Bachelor’s degree from an accredited educational institution preferred (required for DC II, or additional experience)

Responsibilities

Assists participants with activities of daily living (personal hygiene, meal preparation, housekeeping, laundry, money management, etc.).
Transports participants for service delivery needs such as medical appointments, shopping, job interviews, vocational programs, and banking.
Administers medication in accordance with MAP policies and procedures.
Maintains records according to standard formats and timetables, including assessment and progress reports, service notes, incident reports, money disbursements, and other specialized documentation.
Provides for and participates in recreational activities for participants (dining out, spectator sports, movies, etc.).
Participates in the development of treatment plans for those served.
