About The Position

The Center for Human Development (CHD) is seeking a Bilingual Outreach Worker for its Adult Community Clinical Services (ACCS) program in Springfield, MA. ACCS is funded by the Department of Mental Health and utilizes a multidisciplinary outreach approach for individuals needing psychiatric and other services while living independently in the community. This role involves active engagement, including assertive outreach, consistent assessment and treatment planning, skill-building, symptom management, addiction treatment, and monitoring/support for behavioral and physical health.

Requirements

  • High School diploma required
  • Interested candidates should be Bilingual (English/Spanish). With ability to speak, read and write in both fluently

Nice To Haves

  • At least one year experience working with population preferred
  • Bachelor’s degree from an accredited educational institution preferred (required for DC II, or additional experience)

Responsibilities

  • Assists participants with activities of daily living (personal hygiene, meal preparation, housekeeping, laundry, money management, etc.).
  • Transports participants for service delivery needs such as medical appointments, shopping, job interviews, vocational programs, and banking.
  • Administers medication in accordance with MAP policies and procedures.
  • Maintains records according to standard formats and timetables, including assessment and progress reports, service notes, incident reports, money disbursements, and other specialized documentation.
  • Provides for and participates in recreational activities for participants (dining out, spectator sports, movies, etc.).
  • Participates in the development of treatment plans for those served.
