About The Position

We prioritize timely access to services, clinical excellence, and coordinated care across systems. We are seeking a Licensed Intake Coordinator to support our Houston market by leading the intake and assessment process for new clients.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology, or a related behavioral health field
  • Current independent clinical licensure (LPC, LCSW, LMFT, or higher)
  • Minimum 2–3 years of clinical experience, including assessment and diagnosis
  • Bilingual (English/Spanish) required
  • Strong clinical assessment and diagnostic skills
  • Knowledge of medical necessity and behavioral health documentation standards
  • Ability to work independently in community and field-based settings
  • Strong organizational and time management skills
  • Excellent communication and care coordination abilities
  • High level of professionalism, ethics, and confidentiality
  • Proficiency in EHR systems and standard office software

Nice To Haves

  • Experience in community-based or Medicaid-funded behavioral health services
  • Familiarity with trauma-informed care and evidence-based assessment practices

Responsibilities

  • Contact new referrals promptly to initiate services and reduce wait times.
  • Conduct comprehensive diagnostic assessments, including mental status evaluations, risk assessments, and review of clinical history.
  • Formulate accurate diagnoses using DSM-5 criteria and determine medical necessity for services.
  • Identify appropriate levels of care and make referrals when needed.
  • Develop individualized, person-centered treatment plans within required timelines.
  • Ensure all documentation is accurate, complete, and compliant with state, federal, and organizational standards.
  • Maintain timely entry of clinical documentation, including assessments and progress notes.
  • Submit required documentation for clinical review and approval as appropriate.
  • Collaborate with families, support systems, and external providers (e.g., schools, primary care providers, community agencies) to support continuity of care.
  • Coordinate appointments and referrals, including psychiatric or medication management services.
  • Support a smooth transition from intake to ongoing services.
  • Track referrals and maintain accurate intake records.
  • Ensure client demographic and service data are up to date in the electronic health record.
  • Participate in team meetings, trainings, and quality improvement initiatives.
  • Adhere to all organizational policies, procedures, and compliance standards.
  • May provide limited direct services (e.g., therapy, crisis intervention, or skills-based services) within scope of licensure as needed.
