Our goal is to ensure that children with behavioral/emotional needs, and their families, become independent within their own communities. The Intensive Care Coordinator (ICC) position is not only about coordination, it is also assessing and identifying the underlying needs of the youth through regular contact with the family in order to make a direct positive impact in their lives using the Wraparound Service Model. We support families to recognize small areas of progress that builds into major changes. For each family that you work with, you will not only be the key contact for the youth’s providers, you will also be creating an individualized roadmap that guides uniform treatment among the team. As the contact, you will collaborate with multi-disciplinary teams such as PCP, school, DCF, Outpatient therapists and other mental health and/or medical providers.
