Bilingual Health Coach & Educator - Field Based
About The Position
Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Maricopa County, Arizona. In this highly field-based role, you'll build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits, access community resources, and improve their overall well-being. If you enjoy working independently, connecting with people face-to-face, and making a direct impact in your community, we'd love to meet you. Applicants must be comfortable conducting daily in-home visits and traveling throughout Maricopa County, Arizona.
Requirements
- Minimum 1+ year of experience as a wellness health coach or educator
- Current, valid, unrestricted driver's license.
- Reliable personal transportation and ability to travel throughout the assigned territory.
- Ability to maintain automobile insurance in accordance with company requirements.
- Ability to work independently and remotely.
- Strong communication, organizational, time management, and relationship-building skills.
- Proficiency with Microsoft Office (Excel, Word, and PowerPoint).
- Knowledge of health, wellness, nutrition, exercise, and behavior change principles.
- Ability to maintain HIPAA compliance and confidentiality.
- Associate’s degree or two years additional experience in lieu of education.

Responsibilities
- Conduct approximately 6-8 in-home visits daily throughout your assigned service area.
- Provide wellness coaching and health education to support behavior change and healthier lifestyles.
- Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.
- Connect members with community resources and social services.
- Support member engagement, retention, and satisfaction.
- Coordinate care with internal clinical teams and external partners.
- Deliver health education materials to members, caregivers, and families.
