About The Position

Clinical Case Managers on the Re-entry Treatment Team have a unique ability to relate to and motivate clients due to their personal shared reentry experience or closeness with loved ones who have had this experience. They understand how to navigate the distinct challenges that returning citizens face using approaches informed by a strong working knowledge of trauma-informed clinical interventions and community-based resources in Alameda County. Clinical Case Managers focus on providing intensive case management, prioritizing strong collaboration with the clinical team and supporting clients with engagement in services consistently, accompanying clients to appointments, and ensuring they receive necessary services. They will also refer and link participants to housing, employment, and health-related resources in the community. This Clinical Case Manager will be skilled in providing services which result in clients “dealing more effectively with daily problems,” promoting engagement, independence and community reintegration. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture. The RTT Clinical Case Manager reports to the assigned Program Supervisor/ Manager, and works closely with RTT clinicians and trainees, Intake/ Program Coordinator, psychiatry providers, probation officers, county partners, housing providers and other community agencies to coordinate comprehensive client care. This position is covered under the SEIU 1021 Collective Bargaining Agreement. In addition to the established base pay, members will receive negotiated annual increases as follows: January 2027: 3% base pay increase Future increases will be subject to the terms of the collective bargaining agreement.

Requirements

• Proficiency in both verbal and written communication in English and Spanish is required for this position.
• Bachelor’s degree in Social Sciences field (Psychology, Sociology, Social Justice, Social Work, or similar) and 2 years of experience in the mental health field or MA degree in a related field.
• Formerly incarcerated or criminal justice system-impacted (personal or close vicarious experience) highly preferred.
• Experience supporting culturally diverse individuals with severe mental illness and/or substance use disorders is highly preferred.
• Knowledge of or experience with utilizing Motivational Interviewing, trauma-informed care, harm reduction, and rehabilitation interventions.
• Familiarity with Alameda County resources (legal aid, health, employment, housing, etc.).
• Experience in non-profit and community-based organizations preferred.
• Demonstrates and models excellent “customer service” orientation and ability to work with adults of diverse cultural backgrounds.
• Demonstrates clinical knowledge related to mental health disorders and trauma-informed care.
• Demonstrates cultural awareness and provides respectful, client-centered services.
• Exercises extreme discretion with the ability to hold, balance, and transmit public and confidential information needed by various parties.
• Proficient in MS Office and Google Workspace products.
• Valid CA Driver’s license required.

• Accurately record work hours each day by clocking in/out at scheduled start, break, lunch, and end times.
• Take all required meal and rest breaks in compliance with state, federal, and agency policies.

• Notify supervisor promptly of any scheduling conflicts, absences, tardiness, or timekeeping issues.

Responsibilities

• Provide intensive care coordination/case management services for justice-involved adults with serious mental illness (SMI), utilizing recovery-oriented and trauma-informed practices that connect clients to resources, employment support, referrals for psychiatric treatment, linkage to mental health, primary care provider, substance abuse, legal, and life skills services.
• Conduct community-based outreach and engagement activities to establish rapport, promote treatment participation, reduce barriers to care, and support successful community reintegration.
• Monitor client mental health symptoms, behavioral health needs, and risk during community visits and appointments; communicate significant concerns to the treatment team and participate in crisis intervention and conduct safety planning as appropriate.
• Assist clients in increasing motivation to engage in mental health treatment, psychiatric services, medication support and substance use recovery services.
• Support clients in developing organizational, independent living, and self advocacy to improve functioning and long-term stability.
• Coordinate access to medical, dental, psychiatric, substance use, and other health care services and supports while promoting continuity of care.
• Assist clients in navigating, obtaining and maintaining appropriate stable housing, including emergency local shelters, board and care homes, transitional housing and independent living placements in the community by sharing resources and supporting application processes where indicated.
• Coordinate intake appointments including processing medical eligibility checks and tracking timeliness of first and follow up appointments.
• Coordinate referrals and linkage to employment, vocational training, educational opportunities, legal services, transportation resources (bus pass, gas card, etc..), food assistance, and other community-based supports.
• Serve as role model and coach to inspire, engage, and advocate for participants and their family member/caregivers’ rights as they navigate the criminal justice and court systems.
• Consult with and collaborate with program and department Case Managers, Staff Therapists, Supervisors, Director and other agency staff as needed to bring clear and complete information to clients as needed.
• Assess client’s transportation needs and help them establish access to transportation support such as applying for bus passes, para-transit, and other types of transportation assistance.
• Support clients in immediate transportation needs such as driving clients to appointments or to collect basic needs assistance (food, hygiene, safety items).
• Assist clients with locating and accessing needed health care (primary care provider) and dental care in a timely manner.
• Assist clients with obtaining and maintaining public benefits, including SSI/SSDI, General Assistance, Medi-Cal, Calf Fresh, and referrals to SSI Advocate and Substitute Payee services when appropriate.
• Assist in connecting clients needing General Assistance (GA) upon case opening with the goal of them being able to obtain benefits within two months of their case being opened.
• Provide culturally competent support to participants and their family members/caregivers.
• Maintain all records in accordance with the Quality Assurance standards of the Agency.
• Maintain client chart and client record in a database system which includes session progress notes, complete Problem Lists, and ANSAs (Assessment Needs and Strengths Assessments).
• Utilize evidence-based interventions including Motivational Interviewing, trauma-informed approaches, rehabilitation interventions, and recovery-oriented practices to support engagement treatment goals.
• Provide field-based services including home visits, community outreach, accompaniment to appointments, hospital discharge support and other services as clinically appropriate.
• Participate in weekly multidisciplinary collaborative meetings and monthly training sessions.
• Effective and timely team collaboration to meet the needs of our clients.
• Provide relevant and useful linkage and warm hand offs to other community-based providers.
• Submit timely and high-quality documentation, e.g. progress notes, screeners, sub-payee records, monthly and quarterly data for report input and other items as requested.
• Report client progress to relevant team members and supervisor in a timely manner.
• Utilize only approved and work-issued computers to send and receive work-related electronic mail, and access and maintain electronic records and files.
• Use organizational methods and spreadsheets to track client progress and program objectives and key performance indicators included in contract and per Agency standard.
• Other relevant duties as assigned or required.
• Maintain current working knowledge of Alameda County social service resources, housing resources, community resources for right-matched referrals.
• Develop and maintain collaborative positive relationships with community partners to promote timely access to resources.
• Attend relevant training and participate in events relevant to key responsibilities, including case presentations.
• Track and regularly report on key metrics for the responsible function areas.
• Collaborate or lead on projects supporting Agency objectives.
• Understand and apply legal requirements, including confidentiality and risk management.
• Respond via all communication methods (email, phone, Slack messages) in a timely manner
• Maintain flexibility to provide both office-based and field-based services throughout Alameda County.
• Assist with intakes and officer of the day crisis coverage as assigned by program leadership.
