Requirements

  • Active Home Care Aide License.
  • Current Tuberculosis test (TB/TST/PPD tests, if applicable).
  • Valid licensed driver with reliable transportation.
  • Valid state required proof of auto insurance (only required if driving a client).
  • Minimum of one year of documented experience.
  • Ability to read, write, and communicate effectively.
  • Self-directing with the ability to work with little direct supervision.
  • Empathy for the needs of the client.
  • Treat clients, families, staff, and the public with courtesy and respect; presents a positive public image.
  • Works as a team member.
  • Ensures confidentiality and security of the client’s medical information.
  • Travel to client homes/sites within local or regional territory, and/or as needed for various staff and client care services.
  • As directed based on the needs of the business and attending job required training and conferences.
  • Must have reliable transportation.
  • Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
  • Varied assistance is required depending on the ability of the client.
  • Must be able to lift up to 50 lbs (with or without accommodation).
  • Visual and acuity required.

Responsibilities

  • Provide client services as directed by the Director of Operations or the Owners.
  • Provide personal care to the client as applicable, including but not limited to: Bathing and personal hygiene; Grooming, including oral care; Assist the client with dressing and undressing; Assist the client with toileting activities; Perform light housekeeping; Change bed linens and make up the client’s bed; Maintain an orderly client environment; Perform meal preparation and clean up; Assist in feeding the client, if necessary; Feed clients who are unable to feed themselves; Assist client in the self-administration of medication (medication reminders); Support good skin care to prevent skin breakdown (i.e., turn and reposition client; actively encourage movement); Assist client with active and passive range of motions as defined by the plan of care; Provide client with help walking and transferring; Measure and record height and weight; Measure and document intake and output, as required.
  • Assist in providing a safe environment for the client and promote quality client care through adherence to established policies, procedures, and standards.
  • Assist client in walking and transportation to doctor appointments, outdoor activities, shopping.
  • Follow principles of infection control and Universal Precautions.
  • Respond rapidly and appropriately in an emergency situation.
  • Use equipment properly and in a safe manner.
  • Ensure client’s rights are adhered to, including but not limited to: the right to privacy and dignity while care is being provided; the right to accept or refuse care; the right to confidentiality of the client’s information.
  • Provide consistent, reliable attendance and performance of assigned shifts to ensure continuity of care for clients.
  • Demonstrate a consistent focus on client safety, well-being, and adherence to the established plan of care.
  • Understand that dependable and consistent care is an essential function of this position and is critical to maintaining client stability, trust, and safety.
  • Acknowledge that inconsistent attendance, repeated call-offs, tardiness, or failure to follow the plan of care may compromise client safety and place clients at risk.
  • Recognize that inconsistent care or failure to meet reliability expectations is not tolerated, as it directly impacts the safety and quality of care provided to our clients.
