About The Position

This is a part-time position (20 hours) where the Bilingual Case Manager will complete client intake screenings/assessments for Access & Referral programs at Pillars Community Health (PCH), including mental health, substance use disorder, sexual assault, and domestic violence services. The role also provides administrative and operational support for the Access and Referral team.

Requirements

  • Bachelor's degree in nursing, counseling, psychology, clinical social work, marriage and family therapy or related field preferred.
  • At least six months of experience in human services or equivalent combination of training and experience.
  • Customer service experience required.
  • Intermediate computer skills required.
  • Must be Bilingual (English/Spanish) and able to communicate well verbally and in writing in both languages.
  • Must be able to problem solve and make quick decisions during intake calls.

Responsibilities

  • Answers incoming intake calls and returns intake messages within 24 hours.
  • Completes intake screenings and assessments for PCH programs.
  • Uses assessment outcome scores to determine the most appropriate level of care for clients.
  • Provides resources for callers not served by PCH and assists with community connections.
  • Conducts initial benefit checks for clients with Medicaid and Managed Medicaid plans.
  • Gathers and inputs data into electronic department records.
  • Maintains various logs and electronic client records in accordance with department and PCH policies.
  • Outreaches to clients and maintains all logs and tracking systems.
  • Collaborates with other intake staff in processing client requests for service.
  • Works in collaboration with other PCH programs and staff.
  • Participates in organizational meetings, in-services, monthly department meetings, and weekly staff meetings.
  • Coordinates with other community agencies and organizations such as DCFS, schools, hospitals, and other providers.
  • Gains knowledge on different public and private insurance coverage accepted within PCH.
  • Gains understanding of mandated reporting laws and how to act on them under supervisor guidance.
  • May represent Pillars Community Health at external service fairs promoting services.
  • May provide support in the community to help outreach and engage individuals to participate in services.
  • Maintains Excel tracking logs and Carelogic reports.
  • Assists with the coordination, management, and assignment of internal and external referrals.
  • Monitors and manages the department's shared inbox.
  • Performs data consolidation, reconciliation, and cross-verification between Excel records and the Electronic Client Record (ECR) system.
  • Provides administrative and operational support for client transitions and discharges, including waitlist maintenance.
  • Other duties as assigned.
