About The Position

The Access Team serves as the front door to care, ensuring individuals and families experience timely, compassionate, and seamless access to services. The team completes comprehensive assessments, connects clients to the appropriate level of care, and provides interim support to reduce barriers and promote stability while clients await ongoing services. Team members connect clients to internal and community-based resources, including public benefits, housing support, and specialized services, while offering coping-focused mental health support and engagement services such as appointment reminders, rescheduling assistance, and ongoing outreach to ensure successful transitions into therapy. As a Certified Community Behavioral Health Clinic (CCBHC), the Access Team also administers pre, mid and post assessments regarding client care and satisfaction with their services. This position includes eligibility for a monthly performance bonus.

Requirements

  • Bachelors Degree in a Human Services field from an accredited college or university.
  • A valid driver’s license and an insurable driving record.
  • Ability to successfully complete pre-employment screening, including background check, fingerprinting, and drug and TB testing.
  • Bilingual in English and Spanish.

Responsibilities

  • Assist clients in reaching their goals as outlined in treatment plans or through brief, appropriate interventions.
  • Collaborate with therapists and relevant staff to address client needs, coordinate services, and prevent deterioration or hospitalization through consistent monitoring within the programs scope.
  • Refer, coordinate, and assist clients in accessing services such as vocational training, medical care, social security, and housing. Provide aid during application processes and supply supporting documentation as needed.
  • Maintain cooperative relationships with community organizations, public agencies, and private businesses (e.g., landlords, banks) to deliver supportive services. May manage contracts, leases, and other client related paperwork.
  • Facilitate client participation in services and educational opportunities, teach approved psychoeducational curricula, adapt techniques to ensure participant engagement.
  • Educate and guide clients in maintaining daily life activities such as financial management, adherence to medical recommendations, and physical and emotional self-care.
  • Offer support, consultation, and education to family members, caregivers, and other stakeholders to reduce barriers and enhance client outcomes.
  • Ensure engagement by providing flexible service delivery, including home visits, outreach, accompaniment to appointments, and assistance with medication management when clinically indicated.
  • Help clients improve their well-being by coaching behavioral skills, fostering information decision-making, and developing problem-solving and daily living strategies.
  • Track and document client progress toward treatment goals using electronic systems. Record case management notes, correspondence, and performance data to comply with government requirements.
