About The Position

The Case Manager coordinates with other members of the team to assist clients with stabilization, promote self-sufficiency, and facilitate long-term integration. Through case management and resource referral, this position helps clients who have been impacted by complex traumatic experiences meet immediate basic needs and achieve short- and long-term goals. Services are delivered in a culturally responsive manner and often in another language. The Refugee & Asylee programs at LFSRM help people uprooted by persecution and violence work towards self-sufficiency through essential resettlement services including case management, employment, housing, school registration, cultural orientation, and referrals to internal and external services for English language acquisition, health and mental health needs, employment, and economic development. LFSRM works with national and local organizations to provide the services and resources needed for newly arrived Refugees to achieve rapid self-sufficiency, integrate into their new communities, and thrive.

Requirements

  • Must maintain a valid driver's license and carry personal auto liability insurance at the level of $100,000/300,000/100,000.
  • Frequent regional travel. Required to transport clients in personal vehicle.
  • Travel to conduct assigned airport pickups, home visits, or other tasks outside of standard business hours, including evenings, is required.
  • Associates degree OR two (2) years of experience in a related field such as social work, human services, humanitarian, mental health, or other related field. Equivalent combination of education and experience accepted.
  • Must be able to fluently speak, read, translate and write in English.
  • Fluency in a second language is preferred and may be required by the position.

Responsibilities

  • Provide case management: Familiarity with assessment, care planning, facilitation, coordination, and advocacy for support on behalf of and in collaboration with individuals or families.
  • Coordinate services: Knowledge of currently available community resources, services, and supports, and ability to connect individuals or families to appropriate resources to meet their needs through referral, application, or enrollment assistance.
  • Deliver services within diverse cultural communities: Skills and sensitivity in working with individuals and families from a variety of ethnic and cultural backgrounds.
  • Apply Person-Centered Care: Treats individuals as partners in planning, developing and accessing care to ensure services are appropriate to their needs. Puts clients and their caregivers at the heart of all decisions.
  • Meet standards of practice: Familiarity with or ability to follow practice standards, including applicable local, state and federal regulations and guidance, and contractual requirements.
  • Provide crisis intervention: Experience with crisis management, problem solving, and mediation best practices.
  • Communicate professionally: Ability to communicate professionally and cooperate with colleagues, other professionals, community partners, and other individuals involved in the care and coordination of services.
  • Manage case file compliance: Maintain organized, up-to-date physical case files and digital records in accordance with program, agency, and funder requirements.
  • Ensure data accuracy and integrity: Consistently enter, update, and review client data in online systems to ensure records are complete, accurate, and audit ready.
  • Meet documentation timelines: Complete all required documentation within established deadlines, balancing client services with timely data entry, quality and attention to detail.
  • Coordinates with pre-arrival team to prepare for new arrivals and ensure housing is available.
  • Greets clients at the airport, transports to housing, conducts intake and assessment and initiates service plans.
  • Serves as an advocate and resource for clients and provides equal treatment to all client groups in accordance with the agency's commitment to the dignity of all persons.
  • Works collaboratively with other members of the team to assist clients with applications for public benefits, enrolling children in school, obtaining identity documents, and other services and referrals, as needed.
  • Conducts individual and group education and coaching on important life skills including but not limited to accessing transportation, budgeting, and managing household finances.
  • Maintains ongoing communication with clients to assess client progress and provide support as needed throughout the life of the case.
  • Maintains client case notes and documents all client services in a timely and accurate manner, maintaining high quality case file documentation.
