About The Position

Saint Clare's Health is seeking a Behavioral Health Social Worker II to support patients and their significant others across pediatric, adolescent, adult, and older adult populations. The role provides emotional support, crisis intervention, counseling, information and referrals, and assistance connecting patients and families with financial aid and supportive services related to treatment, illness, disability, or trauma.

The Social Worker provides case and milieu management, completes psychosocial assessments, assists with developing and reevaluating treatment plans, and works as part of an interdisciplinary team to coordinate discharge planning. The role also requires completing necessary documentation and maintaining a clinical record for each client throughout their stay.

Requirements

• Master’s degree in Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor, or a related field.
• Where state licensure is required, obtain licensure upon hire and maintain it in accordance with state regulations. In states without a licensure requirement, maintain current registration with the appropriate state board and/or work under clinical supervision to meet clinical hours required for licensure.
• Current BLS certificate upon hire and maintain it.
• Minimum one year of inpatient or outpatient behavioral health experience.
• Experience working with a diverse population and a wide range of problems, such as death and dying, maternal and child care, substance abuse, disability, and chronic illness.
