About The Position

Coordinates and manages patient care and disposition planning for the Inpatient Psychiatric Unit. This role involves coordinating inpatient and post-discharge care, scheduling, and reporting. The Discharge Planner provides disposition planning, maintains disposition data, and collaborates with the interdisciplinary team to ensure continuity of care. They also coordinate available NCH and community resources, provide education and support to patients and their families, and complete all necessary documentation in Epic and other administrative systems.

Requirements

• Bachelor's Degree in social work, counseling, psychology, or a related field, required.
• LSW, required.
• Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives.
• Ability to work with the hospital’s electronic medical records system.
• Ability to work and problem-solve with minimal supervision.
• Ability to adapt to change positively in workflow and process.
• Spreadsheet, data entry and tracking, and computerized scheduling skills.
• Experience providing and managing behavioral healthcare to children, adolescent and families, required.
• Demonstrate ability to pass CCM crisis management trainings once trained.

Responsibilities
