This position provides community-based services to youth and adults with mental health and substance use disorders. The role involves ongoing assessment of needs, developing skills for independence, coordinating treatment plans, symptom monitoring, advocacy, outreach, interventions for symptom management, and promoting recovery. It also includes providing crisis support and stabilization, de-escalating individuals in crisis, facilitating hospitalizations when necessary, assisting with safety planning, and coordinating with treatment teams and after-hours services. The Case Manager may also provide Day Treatment services to adults. Duties and target populations will vary based on site/department needs and will be assigned by the Program Coordinator. All services must be rendered within department guidelines and in compliance with rules and standards set forth by Medicaid/Medicare, HFAP, OhioMHAS, and CARF. A commitment to quality care is demonstrated through professional growth, agency-sponsored learning, utilizing best practices, and providing services that instill hope and meet patients at their readiness for change. The role emphasizes patient engagement and retention, patient safety through screening and risk factor documentation, developing safety and crisis management plans, and coordinating with the treatment team. Accurate and timely clinical documentation is crucial for quality care and patient safety. Assertive outreach is provided to clients not engaging in treatment services, with extensive efforts made for at-risk clients. Patient Care Standards (PCS) are utilized to ensure access to care, and strategies provided through supervision are used to address challenges in meeting these standards. Tasks supporting revenue include timely and accurate documentation and maintaining current treatment plans. Grant or special project tasks may also be assigned. Proficiency in EMR and video-conferencing software is required, with new hires expected to have these skills by the end of their probationary period. All electronic communication involving PHI must be HIPAA and 42 CFR compliant. The role requires positive contributions to patient, referral source, and employee satisfaction, acting as a flexible team player who assists team members and positively influences morale. Active participation in strategic planning, performance improvement, and quality initiatives is expected.
