About The Position

The Bereavement Coordinator will assess hospice families to identify bereavement needs, demonstrate commitment to professional growth and competency, and collaborate with the interdisciplinary team to promote coordination of bereavement care. This role involves providing continued bereavement support, participating in QAPI, training volunteers, and ensuring timely submission of all documentation. The coordinator must also maintain compliance with all relevant laws and regulations.

Requirements

High School Graduate
Degree/License in counseling preferred
Experience in bereavement and grief counseling, preferably in hospice and palliative care setting; grief support counseling
Working knowledge of community resources
Demonstrates good communication, interpersonal, negotiation, and public relations skills
Experience with EMR software is a plus
Current State Driver’s License
Reliable transportation
Provide ongoing valid and current auto liability insurance

Responsibilities

Assess hospice family to identify bereavement needs as evidenced by documentation and community resource referrals
Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards
Collaborate with interdisciplinary team to promote coordination of bereavement care plan and interventions
Provide continued bereavement support to hospice families as evidenced by documentation
Participate in QAPI
Training and monitoring volunteers that support the bereavement program
Timely submission of all documentation
Performance of other duties as required
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
