Requirements

  • Master’s degree in social work or a counseling-related field from an accredited graduate school
  • At least two (2) years of postgraduate experience, preferably in healthcare, hospice, or grief support
  • Excellent communication, interpersonal, and counseling skills
  • Demonstrated ability to support individuals and families experiencing loss and grief
  • Experience designing and facilitating small groups
  • Valid California driver’s license required; reliable insured vehicle or willingness to participate in the company car program
  • Proficiency with computers and documentation systems required
  • Ability to work occasional evenings and weekends as needed
  • Commitment to providing compassionate, patient-centered care

Nice To Haves

  • Prior electronic medical record (EMR) experience preferred
  • Knowledge of trauma-informed care principles preferred

Responsibilities

  • Assist the Bereavement Services Manager in planning, designing, implementing, facilitating, and managing bereavement programs, support groups, and related activities
  • Ensure timely initial bereavement contact with hospice family survivors
  • Coordinate, plan, facilitate, and participate in the Healing Altogether (HAT) program
  • Provide grief education and support through letters, educational materials, individual outreach, support groups, and counseling sessions
  • Respond to inquiries from community members regarding grief support, resources, and referrals
  • Facilitate grief support groups and memorial events, including occasional evenings and weekends
  • Collaborate with the interdisciplinary team to ensure continuity of care and compliance with hospice regulations
  • Maintain accurate and timely documentation in applicable systems
