About The Position

Concierge Home Care is hiring a Behavioral Health Registered Nurse (RN) to support our Sebring territory. This role offers the flexibility and autonomy to provide meaningful 1:1 care to patients in their homes, focusing on mental health and patient independence. The position is full-time with a flexible schedule and operates on a pay-per-visit model, with opportunities for quarterly bonuses and mileage reimbursement. The coverage area includes Orange Park, FL, and surrounding areas, with territories designed to minimize drive time through clustered scheduling.

Requirements

• Active, unencumbered Florida RN license
• Valid driver’s license, reliable transportation, and auto insurance
• Master’s Degree (Psych/Mental Health Nursing) OR Licensed RN with a Master’s specialization in psychiatric/mental health nursing AND Recent experience (within ~3 years preferred) in: Psychiatric hospital, Psychiatric home health, Partial hospitalization program (PHP), or Outpatient psychiatric setting
• Bachelor’s Degree in Nursing (BSN) AND Licensed RN with at least 1 year of recent psychiatric nursing experience (within ~3 years preferred) in: Acute psychiatric care, Psychiatric home care, PHP or outpatient psych services
• Associate Degree or Diploma in Nursing AND Licensed RN with at least 2 years of recent psychiatric nursing experience (within ~3 years preferred) in: Acute psychiatric care, Psychiatric home care, PHP or outpatient psych services
• Must meet qualifications aligned with Medicare home health psychiatric nursing guidelines

Nice To Haves

• Medical/Surgical nursing experience strongly preferred
• Experience with geriatric behavioral health patients
• Ability to work independently while collaborating with a clinical team

Responsibilities

• Provide skilled nursing care with a mix of focused visits on mental and behavioral health
• Perform patient assessments and develop individualized care plans
• Support patients in maintaining safety and independence at home
• Collaborate with physicians and interdisciplinary teams
• Monitor emotional and physical status and adjust care plans as needed
• Communicate updates on patient condition to the care team
• Complete documentation using streamlined, AI-assisted tools
• Deliver care under a physician-established and reviewed Plan of Care (POC)
