About The Position

This position is for a Behavioral Health Therapist who will provide comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, substance use, and social determinants of health needs. The therapist will work collaboratively with a care team to manage patient care, improve self-management of chronic conditions, and detect at-risk patients. The role involves maintaining accurate documentation, adhering to company policies, and contributing to the improvement of behavioral health services at assigned sites. A positive and professional attitude is expected, along with enthusiasm for serving patients.

Requirements

  • Master’s degree in social work or counseling from an accredited graduate school.
  • Licensed as a Licensed or Limited Licensed Master’s Social Worker or Licensed Professional Counselor in the State of Michigan.
  • Basic Life Support (BLS) certification Required.
  • Skilled in written and oral communication.
  • Able to demonstrate positive customer service skills.
  • Basic computer skills and ability to work in an electronic medical record.
  • Able to prioritize activities on a short and long-term basis.
  • Able to handle multiple tasks efficiently and effectively.
  • Functions effectively in the treatment of patients.
  • Able to resolve interpersonal and professional conflicts appropriately.
  • Able to develop and maintain cooperative and effective working relationships with clinical management, members of the executive structure, Board of Directors, business associates, and community groups.
  • If Limited Licensed Master’s Social Worker, must obtain license in a timely manner and be actively pursuing.
  • Be in good standing and continue to be in good standing with expectations regarding licensure.

Nice To Haves

  • Substance Use Disorder (SUD): Michigan Certification Board for Addiction Professionals (MCBAP) certification preferred based upon grant or position specifications.
  • Enrolled in a Development Plan as a Certified Alcohol and Drug Counselor with MCBAP OR additional certification preferred based upon grant or position specifications.
  • Preferred minimum of three (3) years of experience in a behavioral health setting.
  • SUD: Previous substance abuse experience preferred based upon grant or position specifications OR additional experience required based upon grant or position specifications.

Responsibilities

  • Accept referrals regarding patients’ behavioral health needs and respond in a timely and collaborative manner.
  • Provide and document referrals to appropriate internal and external providers/resources.
  • Provide comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, substance use, and social determinants of health needs.
  • Recommend referral to specialty providers or other care team members when assessment indicates.
  • Provide counseling/behavioral health services to patients on an individual, couples, family, and/or group basis.
  • Assist the care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education.
  • Consult and collaborate with patients and the care team regarding recommended behavioral health interventions.
  • Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
  • Use available screening tools, psychological stressors assessment, and diagnostic codes for initial screening.
  • Meet standards for quality and utilization and follow company policies and procedures.
  • Maintain pertinent, accurate, and timely documentation.
  • Monitor behavioral health needs at assigned sites and make recommendations to immediate supervisor.
  • Attend and actively participate in huddles, staff meetings, case conferences, and meetings supporting care models.
  • Display a positive and professional attitude, interact well with colleagues and leadership, and approach the job with enthusiasm.
  • Provide regular supervision to school-based therapists.
