About The Position

The Behavioral Health Therapist II is a fully licensed, dually credentialed Masters level provider who provides clinical behavioral health services and coordination of care to patients, families and referral sources engaged with ProHealth Care. This role involves facilitating urgent assessments, making recommendations for appropriate levels of care, and providing bridge therapy sessions. Referral sources include primary care physicians, specialty clinic providers, and behavioral health providers, as well as referrals for post-acute care follow-up from the Emergency Department and medical floors. The therapist will offer short-term therapy (6 visits or less) when appropriate and facilitate intake evaluations for the PHC Partial Hospital Program. A key aspect of this role is understanding clinical criteria for all levels of care, documenting recommendations, developing positive relationships with referral sources, and identifying community resources for patients whose needs exceed ProHealth Care's offerings. The position also involves utilizing specific billing codes (90971, 90839, 90832, and 90834).

Requirements

  • Masters degree in a related mental health field.
  • Meets state of WI requirements for licensure in accordance with their Masters degree- LCSW, LPC, or LMFT.
  • Meets state of WI requirements for certification as a substance abuse counselor (SAC or CSAC) or Substance Abuse Specialty Authorization added to their licensure.
  • Maintenance of licensure/certifications via completion of appropriate continuing education hours each biennium.

Responsibilities

  • Facilitate urgent assessments and make recommendations for appropriate level of care.
  • Provide bridge therapy sessions to patients within 7 days of discharge from an inpatient psychiatric and partial hospital program.
  • Offer short-term therapy (6 visits or less) if patient is appropriate for that type of service.
  • Facilitate intake evaluations for PHC Partial Hospital Program.
  • Develop and maintain positive relationships with providers with referral sources.
  • Identify and refer to community resources if patient’s needs exceed what ProHealth Care is able to offer.
  • Utilize 90971, 90839, 90832, and 90834 billing codes.
