About The Position

The Behavioral Health Case Manager MS - Inpatient works with the behavioral health multidisciplinary team to provide care for patients admitted to inpatient behavioral health or mental health, addictions, and child/adolescent day treatment programs. This role involves dispositional planning, care planning, and group psychoeducation. The position may also assist with behavioral health consults in the ED and on medical units. Under the supervision of a psychiatrist, the Behavioral Health Case Manager MS - Inpatient will gather patient data, assist with disposition, and provide resources and recommendations to patients in the ED or on medical floors. Other duties as assigned.

Requirements

Master's Degree in a mental health, behavioral health field with documented clinical course work.
Experience or field study experience in mental health site.
Exceptional communication skills

Working toward licensure is preferred.

Responsibilities

Works with patients on the inpatient unit, emergency room, medical floors and day treatment programs.
Consults and coordinates care of patient with the physician/psychiatrist, RN, social worker/therapist and other agencies.
Assesses treatment planning and progress.
Prepares treatment plan and goals for patients.
Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy.
Provides psychoeducational groups.
Arranges for disposition and follow up recommendations.
