About The Position

Cumberland County Guidance Center is seeking a Full-Time PACT Nurse Specialist to join our Program for Assertive Community Treatment (PACT) team in Bridgeton, NJ. This opportunity is ideal for an experienced Psychiatric Nurse, Mental Health Nurse, Behavioral Health Nurse, Community Health Nurse, or Registered Nurse (RN) interested in providing recovery-focused care to adults living with serious and persistent mental illness within the community. As a member of our multidisciplinary PACT team, you will deliver community-based psychiatric nursing services, medication management, health monitoring, patient education, care coordination, and wellness support that empowers individuals to achieve greater independence and improved quality of life.

Requirements

  • Current New Jersey Registered Nurse (RN) License OR Advanced Nursing Degree for Nurse Specialists
  • Minimum two years of experience working with individuals living with serious and persistent mental illness, psychiatric disorders, or chronic mental health conditions
  • Knowledge of behavioral health services, psychiatric medications, and community resources
  • Ability to work collaboratively within a multidisciplinary treatment team
  • Willingness to participate in on-call rotation responsibilities
  • Valid New Jersey driver's license with an acceptable driving record
  • A Background Check is required
  • Clear Five year driving record/abstract with ability to be insured by organizations carrier.

Responsibilities

  • Conduct comprehensive nursing and behavioral health assessments
  • Monitor psychiatric symptoms, physical health conditions, and treatment effectiveness
  • Administer and monitor psychiatric medications
  • Coordinate care with psychiatrists, therapists, social workers, primary care providers, and community agencies
  • Maintain accurate clinical documentation in accordance with regulatory standards
  • Assist clients in accessing medical, psychiatric, housing, and community resources
  • Participate in community outreach, home visits, and crisis support activities as needed
