About The Position

Requirements
- Active RN license.
- Graduate of an accredited nursing program.
- Home health or related behavioral health clinical experience.
- Strong communication, organization, and critical-thinking skills.
- Current CPR certification.
- Valid driver’s license and reliable transportation.

Responsibilities
- Provide direct patient care, including assessments and care plan development.
- Manage a caseload and coordinate care across the clinical team.
- Administer medications and treatments according to physician orders.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and caregivers on care and self-management.
- Communicate with physicians and care teams to ensure continuity of care.
- Supervise and support HHAs and LPNs.
- Complete accurate and timely documentation, including OASIS, visit notes, and orders.
- Ensure compliance with policies, regulations, and HIPAA standards.
