Requirements

• At least 18 years of age
• Must be licensed as an LPCC, LPCC-S, LISW, LISW-S or IMFT
• Master’s degree in social work, professional counseling or marriage and family therapy; license must be maintained during employment.
• Must be Medicare-eligible to provide intake and assessment services
• Possess a minimum of three (3) years of behavioral health, case management, or related clinical experience.
• Must be responsible, mature and flexible; must possess excellent decision-making, time management and communication skills
• Must be able to respond calmly to crisis situations; demonstrate the ability to direct others in a confident, respectful manner and exercise good judgment in interactions with residents, families, employees, peers and regulatory agency personnel.
• Must appropriately handle confidential information.
• Must have a valid driver's license and reliable transportation.

Nice To Haves

• Clinical experience treating individuals with a variety of diagnoses and presenting issues is preferred.
• Experience working in skilled nursing facilities, long-term care settings, geriatric populations, Value-Based Care programs, and/or Behavioral Health Integration (BHI) programs is preferred.
• Experience conducting diagnostic assessments and behavioral health intake preferred.
• Prior team lead, supervisory, or clinical leadership experience is also preferred.
• Bilingual language skills are a plus.

Responsibilities

• Provide clinical oversight
• Intake assessments, conduct behavioral health intakes and psychosocial assessments
• Team resource support within a regional BHI team serving skilled nursing and long-term care (SNF/LTC) settings.
• Support Value-Based Care initiatives, and serve as a clinical Resource escalation point for Behavioral Health Coordinators
• Ideal for clinicians who enjoy systems-level impact, Interdisciplinary collaboration, and shaping a growing behavioral health program.
