About The Position

The Behavioral Health Counselor provides assessment, treatment planning, crisis intervention, and individual behavioral health counseling to patients with mental health and/or substance use disorders. The role involves working with families, collaborating with outpatient treaters and the PHP multidisciplinary team, and leading psycho-educational and support groups.

Requirements

Master’s degree in Mental Health & Behavioral Medicine or Social Work; experience cannot substitute for the degree.
0–1 year of clinical experience in a medical setting required.
Foundational skills in professional values and ethics, formulation, risk assessment, crisis intervention, psychosocial treatment planning, and documentation.
Developing knowledge of internal and external resources and interpersonal collaboration in an inpatient or outpatient medical setting.
Ability to demonstrate fundamental skills under supervision and to seek assistance or guidance as needed.

Responsibilities

Provide patient assessments and develop treatment plans.
Deliver crisis intervention and individual behavioral health counseling.
Work with patients’ families.
Collaborate with outpatient treaters and the PHP multidisciplinary team.
Run psycho-educational and support groups for patients with mental health and/or substance use disorders.
Complete psychosocial treatment plans and appropriate documentation.
Bring patient needs, safety measures, and concerns forward, seeking assistance or guidance when needed.
Participate in department, service, and entity-level initiatives and use data to measure progress.
Collaborate with health care team members and colleagues to support and enhance patient care.
