About The Position

Behavioral Health Counselor II - Per Diem - St. Mary Campus. This is a Per Diem position in behavioral health at SPARC St. Mary Campus, an affiliate of St. Peter's Health Partners. The role focuses on providing behavioral health services as a member of a multidisciplinary team, assisting with monitoring and providing direct support to patients/residents within the scope of practice. The position involves assessment, planning, and implementation of patient care plans through individual and group sessions.

Requirements

High School Diploma/GED and CASAC -T certification is Required.
Minimum 1 years of experience working in an addiction or behavioral related field with skills in planning, leading addiction group therapy, and individual treatment sessions.
Strong writing and communication skills are required.

Nice To Haves

Associate's Degree with CASAC-T preferred.
Knowledge of working with community agencies and managed care representatives is desired.
Experience working with a diverse population and a strong understanding of multicultural issues is preferred.

Responsibilities

Obtains and interprets information about the patient/resident and his/her needs.
May conduct patient education sessions, as assigned.
Observes and records significant changes reporting them to appropriate person(s) immediately.
May conduct psychosocial evaluation and gather appropriate information to support a current DSM diagnosis as assigned.
Assists in screening and/or orientation of new patients/residents to program and program expectations.
Develops treatment/service plan for assigned patients/residents in a timely manner.
Assists with discharge planning.
Plans appropriate activities for assigned patients/residents with team.
Administers care that is consistent with established treatment/service plan.
Conducts patient education groups, as assigned.
