About The Position

The Behavioral Health Coordinator serves as a liaison between the Delaware Division of Medicaid and Medical Assistance (DMMA), the Case Management Team, the Division of Substance Abuse and Mental Health (DSAMH), and behavioral health providers. This role is responsible for coordinating behavioral healthcare services, conducting outreach with members and providers, and maintaining accurate documentation within the electronic case management system. The Behavioral Health Coordinator plays a key role in ensuring integrated, member-centered care and fostering collaboration among healthcare stakeholders.

Requirements

  • Bachelor's degree in Social Work, Behavioral Health, Psychology, Counseling, Human Services, Public Health, or a related field required.
  • Active Delaware Licensed Professional Counselor (LPC) or Licensed Mental Health Counselor (LMHC) license required.
  • Three (3) to five (5) years of professional experience in behavioral health, mental health, substance use disorder treatment, case management, care coordination, or human services required.
  • Proficiency with Microsoft Office Suite

Nice To Haves

  • Master's degree in Social Work, Counseling, Behavioral Health, Psychology, Human Services, Public Health, or a related field preferred.
  • Active Delaware Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license preferred.
  • Master's degree combined with an active Delaware Licensed Clinical Social Worker (LCSW) license is highly preferred.
  • Experience coordinating care among healthcare providers, community agencies, and members preferred.
  • Knowledge of behavioral health and substance use disorder services and provider networks preferred.
  • Experience working with electronic case management, care management, or documentation systems preferred.

Responsibilities

  • Coordinate member care needs with behavioral health providers.
  • Develop and implement processes that enhance collaboration between Primary Care Physicians (PCPs) and behavioral health providers.
  • Participate in identifying and implementing best practices for integrating behavioral healthcare within primary care settings.
  • Coordinate behavioral health services with medically necessary healthcare services to support holistic member care.
  • Maintain knowledge of Promoting Optimal Mental Health for Individuals through Supports and Empowerment (PROMISE) requirements and collaborate with PROMISE providers to ensure compliance and effective service delivery.
  • Utilize knowledge of behavioral health and substance use disorder treatment services to support member care coordination.
  • Develop and maintain strong working relationships with behavioral health and substance use disorder providers across the state.
  • Conduct outreach and engagement activities with members and providers.
  • Accurately document all member and provider interactions within the electronic case management documentation system.
