Requirements

  • Master's or Doctoral Degree
  • Licensed Independent Social Worker (LISW); OR Licensed Professional Clinical Consultant (LPCC); OR Doctor of Psychology (PsyD)/Doctor of Philosophy (PhD)
  • Generalist knowledge of a wide variety behavioral health conditions and evidence based treatment modalities to serve diverse groups of patients across the lifespan
  • Must be able to work in a team based integrated brief treatment model
  • Up to 1 year experience Professional Clinical Social Worker or Clinical Consultant

Nice To Haves

  • Familiar with evidence-based interventions such as motivational interviewing, problem focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy.
  • Familiar with community resources to address patient psychosocial and behavioral health needs.

Responsibilities

  • Provide behavioral health services including assessment and evidence-based treatment in an ambulatory care setting.
  • Administer and score standardized assessment tools, diagnose BH conditions, provide psychotherapy/counseling, and complete supporting documentation.
  • Assist patients in developing skills for disease self-management, including improved coping, distress tolerance, stress reduction, and relaxation.
  • Develop and maintain collaborative relationships with primary care providers, ambulatory practice teams, behavioral health team members, patients, and families.
  • Provide behavioral health education to staff, patients, and families in individual and group settings.
  • Collaborate with community mental health providers and community support services to develop, provide, and support a continuum of care for patients.
  • Provide referral, case management, and follow-up services for patients with behavioral health needs.
  • Engage patients, initiate referrals, address barriers to care, and conduct follow-up.
  • Provide accurate and timely documentation in electronic health records to support patient care.
  • Assist in capturing program evaluation and fidelity measures.
  • Monitor the behavioral health program and identify areas for improvement.
  • Participate in meetings and quality improvement activities, continuing education, and community education.
  • Serve as a member of committees or related activities as directed by a supervisor.
  • May provide supervisory support to other team members, including professional guidance, performance assessment, and approval of intervention plans.
  • Assume responsibility for the welfare of supervisee's clients.
