About The Position

Are you passionate about helping people improve their mental health, physical health, and overall quality of life? Join our team as a Behavioral Health Consultant and become an essential part of an integrated primary care team where behavioral health is not an afterthought—it’s part of the whole-person approach to care. At Cowlitz Family Health Center, you’ll work side-by-side with primary care providers and other healthcare professionals to identify concerns, connect patients with resources, provide brief evidence-based interventions, and help patients take meaningful steps toward better health. From warm handoffs and brief therapy to care coordination and patient education, every day brings an opportunity to make a meaningful difference. If you’re collaborative, adaptable, solution-focused, and thrive in a fast-paced environment where no two patients are exactly alike, we’d love to have you on our team! The Behavioral Health Consultant (BHC) is an integral member of the primary care treatment team, providing integrated behavioral health services to patients within the primary care setting. The BHC works collaboratively with primary care providers and other members of the care team to identify, assess, and address behavioral health, substance use, and psychosocial concerns that may impact a patient’s overall health and well-being. This position provides brief, evidence-based behavioral health interventions, psychoeducation, care coordination, clinical assessment, and ongoing patient support designed to help patients achieve meaningful improvements in their health and quality of life. The BHC may work with patients in person, by telephone, or through video visits, utilizing warm handoffs and other engagement strategies to connect patients with care when they need it most. The ideal candidate is flexible, organized, persistent, compassionate, and highly collaborative, with the ability to prioritize pressing patient needs while adapting to a dynamic primary care environment. The BHC partners closely with providers and community resources to support treatment planning, referrals, recovery, relapse prevention, medication adherence, and continued engagement in care. This is an opportunity to practice at the intersection of behavioral health and primary care, helping patients address the barriers that affect their health while contributing to CFHC’s Patient Centered Medical Home model and commitment to whole-person, community-focused care.
