About The Position

The Behavioral Health Consultant (BHC) position requires an independent license to practice in a health care setting such as a LCSW (licensed clinical social worker) or LCPC (licensed clinical professional counselor). The BHC is a generalist assigned to a community health center, is part of the patients’ care team and works with individuals, groups, and families, of all ages, providing evidence-based, integrated behavioral health care for both mental and physical health conditions. The individual in this position must be comfortable providing telehealth counseling, working in a fast-paced environment, with a flexible schedule, able to complete at least 8 patient visits per day, and adept in working closely with medical providers and other care team members. Other required skills and duties are outlined below.

Requirements

  • Master’s degree in clinical social work, counseling, or clinical psychology; and three years of clinical practice experience in either a hospital, community health or mental health setting providing individual and group counseling.
  • Clinical licensure in Illinois (e.g., LCSW, LCPC) is required.
  • English proficiency required.
  • Excellent written and oral communication skills required.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of individuals or employees of an organization.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Ability to use computers to receive, organize, prioritize, and transmit information. Proficiency in use of Microsoft Office suite including Outlook, Word, Teams, and Excel. Comprehension of and appropriate use of basic medical terminology, application of third-party payer guidelines and concepts related to quality assurance measures. Compliance with organization policies and procedures.

Responsibilities

  • Provide direct patient care individually or in groups, in person or via telehealth, assisting patients achieve therapeutic goals in accordance with their treatment plan, and coordinates care to improve patient outcomes via 30-minute, brief targeted interventions, or short-term counseling and/or screenings.
  • Work as part of an interdisciplinary team to provide the patients’ care team with timely feedback about the patients’ care, treatment recommendations and consultations.
  • Manage the psychosocial aspects of chronic and acute diseases, application of behavioral principles to address lifestyle and health risk issues, consultation and co management in the treatment of mental health and co-morbid disorders and psychosocial issues.
  • See patients immediately after the patients’ appointment with their medical provider through warm hand-offs and develop a plan of action to address the presenting need.
  • Ability to work effectively with a diverse patient and staff population and use of interpreter services as needed.
  • Basic understanding of primary care and nontechnical knowledge of psychotropic and other relevant medications.
  • Provide referrals/resource information on care coordination and access to external programs for patients as needed.
  • Develops and maintains relationships with external providers, staff, and programs.
  • Attends, participates in and reports on various internal and external meetings and trainings.
  • Excellent computer skills and the ability to work in an electronic health record. Maintains documentation of the patient’s treatment by using current and accurate ICD-Coding and CPT coding.
  • Establishes and maintains a professional and therapeutic relationship within the standards of patient profiles, confidentiality laws and the newest clinical research available.
  • Attend trainings, workshops, and other relevant professional development to maintain licensure.
  • Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, organization plans/policies and organization’s guidelines. Participates in periodic evaluative reviews and/or in house and external staff training.
