About The Position

Responsible for receiving, coordinating, and providing services to all Targeted Case Management referrals. Works collaboratively with inter-disciplinary team to assess plan of care, monitor progress and make recommendations for the safest and most efficient community services. Meets and provides interventions for participants in community, at various local agencies, and outpatient practices. Completes referrals, often requiring a LCPC/LCSW, to various agencies for services and benefits the participant may qualify for. Connects the participant with treatment, education, and employment services aimed at improving health and independence. Supports participants to obtain or maintain housing, food security, and reduce incarceration risk. Engages in emergency support and crisis services in community when required.

Requirements

  • Currently licensed as LCSW/LCPC in the state of Maryland.
  • Experience in Psychiatric Case Management.
  • Knowledge of resources in Harford County.
  • Basic computer skills required.
  • Demonstrate skill in psychiatric clinical case management and assessment of psychiatric patients.
  • Demonstrate skill in performing complete biopsychosocial assessments.
  • Demonstrate skill in effective critical thinking.
  • Demonstrate skill in written and oral communications.
  • Demonstrate skill in age-appropriate interpersonal interactions.
  • Demonstrate knowledge of Medicaid regulations.
  • Demonstrate knowledge of acute care criteria.
  • Demonstrate knowledge of case management principles.
  • Demonstrate ability to communicate and collaborate effectively with both internal and external customers.
  • Demonstrate ability to assess, adapt, and calmly respond to changing and/or crisis environment.
  • Demonstrate ability to make independent decisions consistent with current policies, procedures, and ethical standards.
  • Demonstrate ability to prioritize work assignments and manage time effectively to complete duties.
  • Be available to adapt hours to meet the needs of all customers.
  • Requires knowledge and use of the appropriate computer systems to communicate effectively with both internal and external customers.

Nice To Haves

  • Experience in Critical Diagnosing preferred.
