About The Position

Community Health Care – North Canton is seeking a full-time (40 hours per week) Clinical Therapist (LPCC or LISW) to provide behavioral health services in an integrated setting with multi-disciplinary care teams in a primary care setting. The role requires proficiency in providing therapeutic services to a diverse patient population ranging from pediatrics (5+) through geriatrics. The office environment is described as energetic, team-oriented, and dedicated to excellent patient-centered care.

Requirements

LPCC, LPCC-S, LISW or LISW-S
Comprehensive knowledge of area resources
Ability to effectively engage patients of all ages in a therapeutic relationship
Strong verbal and written communication skills including the ability to work with patients by telephone and virtual visits, as well as in person

Responsibilities

Work within a care team while providing direct counseling services to the patient
Assess patients for common mental health disorders using evidence-based tools
Provide brief behavioral interventions using evidence-based techniques such as cognitive behavioral therapy, problem-solving treatment, motivational interviewing, or other treatments as appropriate
Organize patient outreach to facilitate follow-up care or services
