As a Behavioral Health Clinical Team Manager, you’ll play a key role in guiding a compassionate and skilled team to deliver exceptional home-based care. Requirements

• Associate’s Degree in Nursing (Bachelor’s preferred).
• At least 2 years as a Registered Nurse in Home Health or Behavioral Health, with 1+ year in a leadership role.
• Familiarity with enterprise systems (Workday, HomeCare HomeBase) and MS Office Suite.
• Current state RN license.

Responsibilities

• Oversee and coordinate home health services tailored to behavioral health needs.
• Manage patient and provider assignments, ensure compliance with regulations, and prioritize timely starts of care.
• Approve and evaluate individualized care plans to maintain the highest clinical standards.
• Seamlessly integrate and coordinate physician orders.
• Inspire and support a team of healthcare providers, ensuring patient needs are continuously assessed and met with excellence.
• Manage processes for physician orders, staff communication, and interdisciplinary coordination for holistic care.
