The Case Manager II, Behavioral Health, advances the department’s mission by providing experienced referral management and case coordination for patients with behavioral health needs. The Case Manager II, manages and tracks referrals, coordinates communication between providers, and ensures timely linkage to care. The Case Manager II also documents all referral activities, assists with more complex patient navigation, and supports the team in addressing barriers to care.
