About The Position

This is a field-based role for the Manhattan area, with the possibility of virtual work from home if not in the field. The position requires travel within the assigned area to client sites or designated locations, with occasional office attendance for meetings or training. The role involves performing case management telephonically and/or through home visits for members with behavioral health and substance abuse needs. It also includes acting as a hospital liaison, visiting inpatient psychiatric, substance abuse, and detox units. The case manager will respond to complex cases, use screening criteria and clinical judgment to assess member needs, develop and monitor care plans, coordinate with internal and external resources, and serve as a resource to other case managers. Participation in cross-functional teams, projects, and initiatives is also expected.

Requirements

  • MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing.
  • Minimum of 3 years of clinical experience in social work counseling with a broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
  • Current active unrestricted license such as LCSW, LMHC, LPC, LMFT, LMSW, or Clinical Psychologist to practice as a health professional issued by the state of New York.
  • Previous experience in case management and telephonic and/or in-person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders.
  • Managed care experience required.

Nice To Haves

  • Experience with care coordination and discharge planning is strongly preferred.
  • Experience in health coaching and motivational interviewing techniques preferred.

Responsibilities

  • Perform case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.
  • Serve as a hospital liaison, visiting inpatient psychiatric units, inpatient substance abuse units, and detox units.
  • Respond to more complex cases and account-specific requests.
  • Use appropriate screening criteria knowledge and clinical judgment to assess member needs.
  • Conduct assessments to identify individual needs and develop specific care plans to address objectives and goals.
  • Monitor and evaluate the effectiveness of care plans and modify them as needed.
  • Support member access to appropriate quality and cost-effective care.
  • Coordinate with internal and external resources to meet identified member needs and collaborate with providers.
  • Serve as a resource to other BH Case Managers.
  • Participate in cross-functional teams, projects, and initiatives.
