About The Position

EverCare Mobile Health is a community-based behavioral health agency focused on improving mental health, independence, and quality of life for individuals living in residential and long-term care environments. We partner with independent living communities, assisted living facilities, skilled nursing facilities, memory care communities, group homes, Homes for Adult Rehabilitation Treatment (HART), and other residential care settings. By bringing behavioral health services directly to clients where they live, we help strengthen communication among residents, families, facility staff, and behavioral health providers. Requirements

• Bachelor's degree or higher in a health or human services field from an accredited institution. Relevant fields may include social work, psychology, sociology, human services, behavioral science, counseling, or another qualifying health/human-services discipline.
• Able to meet current Idaho Behavioral Health Plan/Magellan credentialing and supervisory-protocol requirements applicable to the services provided.
• Ability and willingness to participate in ongoing clinical supervision.
• Ability to successfully complete the applicable Idaho Department of Health and Welfare criminal history and background-check process, including fingerprinting and enhanced clearance when required. Idaho DHW currently identifies behavioral health programs and mental health services among programs subject to enhanced background-check requirements.
• Valid driver's license and reliable transportation appropriate for regular travel between client locations.
• Current automobile insurance meeting company requirements when using a personal vehicle for business purposes.
• Ability to maintain confidential client information and comply with HIPAA and company privacy requirements.
• Strong written communication skills and ability to complete timely, accurate electronic documentation.
• Ability to work independently in residential and community settings while maintaining regular communication with supervisors and the interdisciplinary care team.

Nice To Haves

• Previous experience in behavioral health, mental health case management, CBRS/skills building, social services, community-based services, or residential/long-term care.
• Experience working with adults or older adults experiencing serious mental illness, functional impairment, or complex medical/social needs.
• Experience working in assisted living, skilled nursing, memory care, group-home, HART, or other residential settings.
• Experience using an electronic health record (EHR).
• Current CPR/First Aid certification preferred
• Familiarity with Idaho Medicaid and the Idaho Behavioral Health Plan.

Responsibilities

• Provide Community-Based Rehabilitation Services (CBRS)/Skills Building consistent with each client's individualized treatment goals and authorized services.
• Help clients learn, practice, and apply functional skills related to coping, communication, social functioning, symptom management, independent living, problem solving, and other identified needs.
• Deliver CBRS primarily in person and within the client's natural living environment, consistent with current payer requirements and clinical direction.
• Coordinate closely with the client's treating clinician so that CBRS supports and reinforces clinical behavioral health treatment rather than functioning as a stand-alone service.
• Provide case-management services including identifying needs, connecting clients with appropriate resources, coordinating services, assisting with transitions of care, advocating for client needs, and monitoring whether services remain appropriate.
• Collaborate with counselors, psychiatric providers, facility staff, primary care providers, families or guardians, and other members of the client's care team when authorized and appropriate.
• Participate in individualized treatment and service planning within the scope of the position and under required supervision.
• Monitor client progress toward functional and service-plan goals and communicate significant changes or concerns to the supervising clinician.
• Complete accurate, timely progress notes, case-management records, service documentation, and other required records in accordance with payer, Medicaid, Magellan/IBHP, HIPAA, and company requirements.
• Participate in required treatment-plan reviews, care-team meetings, staff meetings, training, and supervision.
• Monitor service authorizations and communicate potential authorization, documentation, or continuity-of-care concerns to the appropriate team member.
• Recognize potential behavioral health or safety emergencies, follow EverCare crisis procedures, and promptly escalate concerns to the appropriate clinical or emergency resource.
• Maintain professional boundaries and provide respectful, person-centered, culturally responsive, and nonjudgmental services.
• Protect client confidentiality and comply with applicable privacy and information-security requirements.
