About The Position

Sentara Health Plans is hiring for a Behavioral Health Care Coordinator/RN/LPC/LCSW/LMFT - Whole Person Care for Residential Facilities- Hybrid in the Central Virginia (Richmond and Petersburg, VA). This is a full-time, permanent position (40 hours/week) that is hybrid, based in Central Virginia (Richmond, Petersburg). The role includes travel of up to approximately 2 days per week in the residential facilities in Central Virginia (Richmond, Petersburg) and 3 days working from home. The position will work with members under Sentara Health Plans who are both medically and behaviorally complex, including children, adolescents, and adults.

Requirements

  • Master’s degree in Counseling, Social Work, or Marriage and Family Therapy required. For Registered Nurses, a Bachelor of Science in nursing is required.
  • Valid license in state of practice. One of the following qualifies: Licensed Clinical Social Worker (LCSW) - Virginia or North Carolina; Licensed Professional Counselor (LPC) - Virginia or North Carolina; Licensed Marriage and Family Therapist (LMFT) - Virginia or North Carolina; Registered Nurse (RN) - Virginia or North Carolina.
  • 2 years of behavioral health experience.
  • 1 year of case management experience.

Nice To Haves

  • Experience in a residential facility is preferred.
