About The Position

Sentara Health Plans is hiring for a Behavioral Health Care Coordinator/RN/LPC/LCSW/LMFT - Whole Person Care for Residential Facilities- Hybrid in the Central Virginia (Richmond and Petersburg, VA)! This is a full-time, permanent position working 40 hours per week. This is a hybrid role based in Central Virginia (Richmond, Petersburg), involving approximately 2 days per week travel to residential facilities and 3 days working from home. The position will work with members under Sentara Health Plans who are both medically and behaviorally complex, including children, adolescents, and adults.

Requirements

  • Master’s degree in Counseling, Social Work, or Marriage and Family Therapy required.
  • For Registered Nurses, a Bachelor of Science in nursing is required.
  • Valid license in state of practice.
  • One of the following licenses: Licensed Clinical Social Worker (LCSW) in Virginia or North Carolina, Licensed Professional Counselor (LPC) in Virginia or North Carolina, Licensed Marriage and Family Therapist (LMFT) in Virginia or North Carolina, or Registered Nurse (RN) license in Virginia or North Carolina (single or multi-state).
  • 2 years of behavioral health experience (Required)
  • 1 year of case management experience (Required)

Nice To Haves

  • Experience in a residential facility is preferred.

Responsibilities

  • Work with members under Sentara Health Plans who are both medically and behaviorally complex.
  • Serve a population including children, adolescents, and adults.
  • Travel to residential facilities in Central Virginia (Richmond, Petersburg) approximately 2 days per week.
  • Work from home 3 days per week.
