About The Position
Centurion is seeking a part-time Behavioral Health Case Manager to join their team at the Mark Luttrell Transition Center in Memphis, Tennessee. This role is responsible for assessing, planning, and coordinating transitional services for offenders, focusing on their re-integration into the community. The Case Manager will work with offenders requiring clinical services or placement upon release to ensure continuity of care.
Requirements
- Bachelor’s degree in social work, human services, counseling or closely related field
- One to three years of experience in social casework in a hospital, community treatment center or other community services program
- Current CPR and Basic Life Support Certification
- Must be able to pass background investigation and obtain agency security clearance
Nice To Haves
- Experience in a correctional environment preferred
Responsibilities
- Assessment, planning and coordination of transitional services for offenders.
- Working with offenders regarding their re-integration from a correctional facility to the community.
- Ensuring continuity of care for offenders requiring clinical services or clinical placement upon release.
