What You Can Expect: Will conduct assessments, develop and provide ongoing evaluation and monitoring of individualized behavior plans, provide training to staff on best practices, participate in case conferences, and collaborate with families and caregivers Will participate on the patient’s care team helping guide and direct appropriate levels of care Support coordinating, scheduling, communicating and continually evaluating the effectiveness of functional assessments, behavioral evaluations and plan for patients. Provide training to family, caregivers and support staff on care plan Develop, monitor and adjust strategies for treatment plan as needed Full Time, weekday schedule Fully benefitted with hospital contributions
