About The Position

Sentara Health is looking to hire a Behavior Health Utilization Management Care Coordinator. This position will work with members under Sentara Health Plans who are both medically and behaviorally complex. This position requires both in-person/face-to-face assessments and remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+) and Substance Abuse Disorders. Applicants must be in Winchester, Stephens City, Leesburg, Fairfax, Chantilly, Front Royal, and Alexandria, VA. Travel may include up to 90 minutes one way.

Requirements

Bachelor’s degree
Valid license in state of practice: LCSW, LPC, LMFT, LSTAP, or RN
2 years of behavioral health experience
1 year of case management experience

Responsibilities

Conduct in-person/face-to-face assessments for members.
Conduct remote telephonic assessments for members.
Work with members who are medically and behaviorally complex.
Work with members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+), and Substance Abuse Disorders.
